Important Information Before Applying

Please complete all required sections of this private enrollment form to apply for childcare at MET-EIS Hoikuen. The information you provide will be used to process your application and support safe and appropriate childcare.

This form includes child and family information, requested childcare schedules, health and medical information, emergency contacts, consent choices, supporting documents, and the Parent Agreement.

Additional information or documents may be requested during the enrollment review process. MET-EIS will provide instructions on how to submit any additional documents securely.

Submitting this form does not guarantee enrollment. Enrollment will be confirmed only after MET-EIS has reviewed the application, required documents, available space, staffing arrangements, requested childcare schedule, and payment arrangements.

Current Subsidy Notice

MET-EIS Hoikuen is currently not eligible for Japan’s free early childhood education and childcare subsidy program. Childcare fees are therefore generally the responsibility of the family. Families may contact their local municipality to ask whether other childcare support or subsidies may apply.
